Example scenarios: service, health and real estate

Processes where the person filling in the form is a customer or a patient, not an employee of the organisation. Each scenario is shown as an illustrative example, not a customer story.

Example scenario

  • Organisation type: Clinic or medical practice
  • Area: Healthcare
  • The process: Informed consent form
Illustration of an informed consent form process in ZeeForm

The situation today

The form is printed, filled in by hand in the waiting room, scanned into the file and is sometimes barely legible. If a field is missing, it's discovered only after the patient has left.

What it looks like in ZeeForm

The process consists of:

  • A link sent by SMS or email before the visit

  • A responsive Hebrew form that opens properly on a phone, no zooming

  • Digital signature and a signed document produced on the server

What changes

The process consists of:

  • The patient fills in and signs before arriving instead of in the waiting room

  • The information arrives structured, not as a picture of a page

  • A missing required field is caught while filling in, not after the patient has left

A note on these scenarios

The scenarios on this page are illustrative examples, not customer stories. They describe real processes that are common in organisations, and how they look in ZeeForm. They contain no customer names and no measured results.
